Application for membership in the St. Louis Physician Alliance is open at this time to any board-certified or board-eligible physician wishing to join the network and its clinical partners in an accountable alliance to improve quality, reduce costs, and clinically integrate care of our patients.

To apply for membership, complete the application form at the link below and return it to us as instructed on the form. Once received, we will send you a copy of the participation agreement for review and signature. The physician Board of Managers reviews all applicants for membership. You will be invoiced for the one-time $300 participation fee per physician. There are no other costs or fees associated with membership.

Application form for St. Louis Physician Alliance membership

Participation in our accountable care organization, SLPA ACO, requires a separate participation agreement and is only available during the annual CMS enrollment window during the summer for the following performance year. You must be a member in good standing of the St. Louis Physician Alliance in order to apply for participation in SLPA ACO. There are no fees associated with ACO participation.
